Required Documents for ALL Student Athletes

The following documents are required of each student (including athletes, managers, athletic trainers, statisticians, filmers, etc.) participating in Athletics each school year.  Every document must be completed in full and returned to the appropriate coach prior to the athlete participating in any practice, scrimmage, tournament, or game.

Athletic Participation Fee

All students participating in Athletics will be required to pay a $50 participation fee annually.  The fee will be collected one time, regardless of the number of sports or teams a student participates in per school year.  

We will begin collecting the fees after the charge is assessed on the student's Skyward account after Sept. 1, 2024.  Fees will be paid online through Skyward.

Exemptions for this fee will be based on the free and reduced lunch program qualifications.

All (10) pages of this form must be completed by parent/guardian, student and licensed health care provider EACH year of activity participation.

THIS FORM MUST BE ON FILE IN THE ATHLETIC TRAINING OFFICE BEFORE the Student WILL BE ALLOWED TO PARTICIPATE IN ANY TRY OUT, PRACTICE, SCRIMMAGE OR GAME.

All physicals must be on the Current SFISD form and performed after 4/30/2025 in order to be current for the 2025-2026 school year

The University Interscholastic League exists to provide educational extracurricular academic, athletic, and music contests. The UIL was created by The University of Texas at Austin in 1910 and has grown into the largest inter-school organization of its kind in the world.

UIL Previous Athletic Participation Form (PAPF)

The Previous Athletic Participation Form must be completed when a student-athlete registers with SFISD to play athletics. This form must be completed and signed by all appropriate parties BEFORE the student can participate in any games or contests. Once the student and parent/guardian have completed their portion, the form should be delivered to the SFISD Athletic Department for review and submitted to the appropriate parties.  The head coach will be notified when the student athlete has been cleared by the District Executive Chairperson.
